Admissions Information
To be admitted to one of our inpatient programs, individuals need to obtain a psychiatric evaluation and medical clearance by an emergency room and community psychiatric crisis team. Call us for more information on the necessary steps needed prior to an admission.

If you have a patient who is in crisis, please seek immediate care at a local hospital emergency room or call 911.
Medical Record Requests
Requests must be accompanied by the consent of the patient or a legally authorized person. Please email, mail or fax the completed medical records release form to the medical records department and allow five to seven days for processing.
